Insurance Coverage

Many insurance companies do cover acupuncture and certain diagnostic lab work and imaging, but policies vary greatly in terms of referral requirements, the number and types of treatments/services covered, deductibles, co-pays or percentage of coverage.

Medi-Cal/Partnership Health Plan of California

Medi-cal/Partnership Health plan of California covers acupuncture treatments for certain conditions. In most cases, a "RAF" will have to be sent by your primary care provider. If you are a "special" or "direct" member, a RAF may not be needed before establishing care and receiving treatments. Please contact us to verify your membership.   

 

Dr. Fialk is also a Medi-cal approved ordering, prescribing and referring provider and is able to prescribe certain medications, order standard lab work, and refer to participating specialists if needed.

Medicare Advantage

Acupuncture treatments for chronic low back pain are covered by some Medicare advantage/supplemental plans. Please contact us to verify your benefits and eligibility.

Discounted Time of Service Rates

Discounted time-of-service rates are available for those who are uninsured or under-insured. We accept cash, checks, credit cards (Visa and Mastercard), Health Savings Account cards (HSA) and Flexible Spending Accounts cards (FSA).
